This issue has been addressed at 10.4 Final. At 10.3.1, there are two approaches to work around the issue, as document in this article: http://support.esri.com/en/technical-article/000012547 1) Applications created using the workflow My Content > Create > App > Using a Template do not experience this issue. 2) To fix applications created using the Map Viewer > Share > Make a web Application workflow, open the items details page for the web application and edit the URL property to include the web adaptor name.
