Two related records are created by Collector for ArcGIS when two users with separate devices make offline edits to the same feature, despite the one-to-one (1:1) cardinality.

This is expected behavior when editing the exact same features in separate offline areas where each user adds a new related record. While the application for each offline area only allows the user to add only a single related record for 1-to-1 relationships, through the individual sync processes for each area, the related records will be added to the same table.
For these scenarios, here are some recommendations:
- if the users are required to edit similar features, change the relationship cardinality to one-to-many.
- if the users are not required to edit similar features, create unique offline area extents to separate the features. This is possible through generating offline areas though the web map -https://doc.arcgis.com/en/arcgis-online/manage-data/take-maps-offline.htm
- If multiple users work in similar areas, control the visibility of features through the use of editor tracking with ownership-based access enabled.
