The Metadata Importer geoprocessing tool and the Import Metadata tool do not import the metadata from a personal geodatabase (.mdb) feature class to the metadata of a .mxd.
Duplicate of NIM089163: ArcMap map documents and layer files require users to explicitly create metadata for these items using the metadata editor. Once the metadata has been explicitly created for those items, users can import metadata to them.

Open ArcMap 10.6.1.
In the Catalogpane, right-click and select Item Description.
In the Item Descriptiondialog box, click Save. This saves the .xml document for ArcMap.
Use either the Metadata Importergeoprocessing tool or Import Metadatatool.
The Import Metadata tool
Reopen the metadata for the .mxd and the feature class within the personal geodatabase. Please note that the metadata for the .mxd and feature class do not match. An error message stating similar to what is found the Import Metadata attachment is received.
In the CatalogPane, select a .mxd document and click Import.
In the Import Metadatatool, choose the feature class as the Source Metadata.
Select any Import Type(Note: Each Import type resulted in the same problem).
In the Target Metadata, select the .mxd document.
In the Catalog pane, expand the personal geodatabase and select a feature class. Right-click and select item description. Insert any text in the Summary, Description, Credits, Use Limitations, and other fields.

In the Target Metadata, insert the .mxd document. An error message stating similar to the Metadata Importerattachment is attached.
