The display and output engine in ArcMap is based on Windows GDI, which is limited to integer coordinates and calculations. Because of this, some inaccuracies can occur in these calculations, and result in small errors.
This is not entirely avoidable, but error can be minimized by requesting output at a size that's divisible by 72 - this is outlined in this support article, http://support.esri.com/technical-article/000008103
One suggested workaround is to complete the cartography in ArcMap, import the MXD into ArcGIS Pro, and use ArcGIS Pro to create the output. Because ArcGIS Pro does not depend on GDI, these rounding errors should no longer occur.
