If a small visibility scale range is fixed for a feature layer, the Filter widget is not able to identify the total map extent when it is turned off in ArcGIS Web AppBuilder.

This is not a BUG, but rather by design.
If the option 'Zoom to the initial map extent when layer's filters are all off' is not checked, then depending on the remaining features and their visibility, the ArcGIS Web AppBuilder app calculates an optimal extent to zoom out to. In the cases where no features are visible, it will automatically zoom-in a level so that the features become visible for a better user experience which is why it is not expected to show the default extent.
解决办法
Enable the 'Zoom to the initial map extent when layer's filters are all off' option in the Filter widget configurations.
Leave all the full range of scales in the Visibility setting.
