This issue is non-reproducible with the latest ArcGIS Field Maps version. For the Field Maps mobile app, the recommended approach is to use Field Maps Designer to configure the form. The Level_ID field is typically hidden because it is automatically populated based on the floor filter selection in the mobile app and there is no need for an Arcade expression.
