Erro
When values of 0 are used on the 'choices' tab in the XLSForm, the option is selected by default in the form if a survey is constructed using the select_one question type, which is selected by default, especially when using a repeat and boolean values as an option.

Solução Provisória
By mistake, the empty values are interpreted by ArcGIS Survey123 as 0. To solve the problem, do not use 0 in the choice_list values.
