Solução Provisória

Create a Basemap Group in Portal for ArcGIS.
Create a web map in Portal for ArcGIS with just the custom basemap > Share with the Basemap Group.
Navigate to My Organization > Edit Settings > Map > Basemap Gallery > Select your Basemap Group> Add default basemaps > Save.
Create a map in Portal for ArcGIS with other layers > Save > Share as Application > Option to use custom scale basemap from the Basemap widget.
