Erro
In ArcGIS Server 10.6, cached map services preview at the full extent of the service in the ArcGIS API for JavaScript preview link from REST, while in earlier versions of ArcGIS Server cached map services preview at the extent used in ArcMap when publishing the service.

Solução Provisória
Set a custom full extent in ArcMap for the MXD, so that it is zoomed in far enough to see features. Publishing with that MXD shows features in the Java Script Viewer in the initial extent.
Thumbnail Workaround:
Create customer thumbnails in ArcCatalog > Description > Edit
