This issue is fixed in ArcGIS Survey123 3.19.

Solução Provisória

This bug occurs when the installed ArcGIS Survey123 website is configured to use ArcGIS Maps SDK for JavaScript 4.20 shipped with ArcGIS Enterprise 10.9.1 or earlier.

The best workaround is to configure it with ArcGIS Map SDK for JavaScript 4.21 or later (for example, ArcGIS Enterprise 11.0 ships with ArcGIS Map SDK for JavaScript 4.23). If an ArcGIS Enterprise update is not possible and in a disconnected environment (for example, unable to use https://js.arcgis.com/4.xx/init.js), the ArcGIS Map SDK for JavaScript 4.21+ can be installed independently and referenced in the ArcGIS Survey123 website installer's config.json file (or during post-installation).
