This is as designed. When setting up a visibility rule, there is an option to allow submitting the answer even when the visibility rule is not met. For example, default values are always submitted regardless of the visibility rule and can later be edited as existing data. This can be configured using Web Designer or XLSForm:
Using Web Designer
- Select the integer question > Behavior > Visible > Set rule.
- When the visibility rule is not met, choose Submit the answer.
Using XLSForm
- Use the body::esri:visible column.
