Surveys published using the ArcGIS Survey123 web designer that contain a page with multiple columns will only display a blank page when opened in the ArcGIS Survey123 field app.

対処法
If publishing in the ArcGIS Survey123 Connect is an option, download the survey in ArcGIS Survey123 Connect. Open the XLSX form, delete rows 2-7, then save and republish the survey.
