When editing a survey with an existing feature class appended to it, there is unusual behavior with 'select_one', and the relevant questions within a repeat group.

Option 1:

Delete the first repeat , add the second repeat, and submit the survey. When editing the record for the survey again, the correct format is shown. The issue with this workaround is, if there are a lot of points (records), this process would have to be done for each point.

Before:

Select_One shows as the plus sign (Image 7)

When an option is chosen, relevant questions do not show (Image 8)

After:

Select_one question properly shows (Image 9)

Relevant questions properly show (Image 10)

Option 2:

Do not use the repeat option. Instead, if there are additional attachments, use the select_one option to request the user for additional attachments. If Yes, then show the questions again using the Relevant Property. See attached .xlsx file called Workaround_TestAttachments for example.
