Publishing a survey from the ArcGIS Survey123 web app returns the error message, “999: The survey element named 'generated_note_form_title' has no label or hint” if survey elements are disabled.

To hide the header, footer and description when sharing a survey:

1. Publish the survey while keeping the header, footer and description enabled.
2. Include "?hide=header,footer,description" at the end of the survey URL (for example - https://survey123.arcgis.com/share/104cbea2c8fd4b72887aeb562dbeb80e?hide=header,footer,description).
