Bogue
Fields in smart forms disappear after refreshing the ArcGIS Field Maps web application when the layer is created in ArcGIS Online using the 'Define your own layer' option.

After refreshing the page, the field shows up under the Field list. From there, click the 'Add all' button to add the field and save it again.
