Auto-populated date format answers in the Survey123 for ArcGIS web app does not appear the same as in the field app.

The format-date function works best with a date, time or dateTime input. If users need a custom formatted date or time value stored as a string, the best approach is to use a hidden (or dateTime) field with the now() function in the calculation column, followed by a text question with the format-date function. Here is an example of how this would look in the XLSForm using the example provided.
