Based on the provided information, the description matches the expected behavior of ArcGIS Pro for working with packages. When you open a package from the active portal the first time, the package is downloaded and unpackaged. If you attempt to open the same package again from the portal and the portal item has not been updated, the package file is not downloaded and unpackaged a second time. You must delete the local cached version of the unpacked package at \Documents\ArcGIS\Packages to get a new version of the original package. All packages work in this manner, not just project packages.
However, if the portal item was updated since the last time the package was accessed, the current version of the package is downloaded and unpacked when you try to use the package from the portal item. You will now have two versions of the package on the local machine: the old version and the new version. If you might still have a reference to the old version, which will still be used if that is the reference you have. If you have a reference to the new version, that one will be used. Each version of the package has a different GUID identifier. i.e. for projects, there might be two entries in the recent projects list, one that points to the old project and one that points to the new one.
