This issue is not reproducible. Versions 3.17, 3.18, and 3.19 of the ArcGIS Survey123 website installer are tested. The 'uploadForms.zip' package (containing the templates to use in a disconnected environment) was updated in January 2024, which may have mitigated this problem.
