The rating for an item in Portal for ArcGIS decrements by 1 when the item is rated either 5 or 4 stars.

Portal and Online both use a Bayesian averaging algorithm for ranking items where : Bayesian Rating = (Average Number of Votes across all item * Average Rating of all items) + (Total People Who Rated * Item’s Total Rating) / (Average Number of Votes across all items + Total People Who Rated) When the user selects a rating of 5 stars this value is applied to the averaging algorithm which is why you see the rating change.
