Solución alternativa
1. Do not stack elements but convert one element at a time into features and then add them to AnnotationLayer. Refer source code in the CreateAnnoTool_ESRI_Workaround_I.cs file.2. Reverse the order of elements in the stack, after the second element, so that in ArcMap view opaque circles cover the text elements.
