Set the Minimum reference scale of the layers in the map document to be 1:1, to force the CachingControllers geoprocessing Service to cache starting at the L00 level.
If set the Maximum reference scale to be very large (e.g. 1:1,000,000,000), there is no guarantee that L23 (from the ArcGIS Online template) will be cached. Instead what seems to happen is that the highest Level cache tile that makes 'sense' for ArcGIS Server will be cached.
E.g. for a metro area, that might be L14 or L15 whereas for a country the size of the contiguous US that might include up to L20 or even higher.
From the Catalog pane, right-click the map document, select Properties > General> and click enable 'Draw up to and including the maximum scale in scale ranges'.
