This is a known limitation in ArcGIS Enterprise 10.8.1 when creating a smart form that contains contingent value field groups. The individual fields that comprise those field groups must be configured with the 'combo box' input type for the form to be fully supported within the ArcGIS Field Maps mobile application. However, the 'combo-box' input type is not supported in the ArcGIS Field Maps web application opened in Portal for ArcGIS version 10.8.1.

Solución alternativa

For the form to be supported within the ArcGIS Field Maps mobile application, the contingent value fields must be configured in the form to use the 'combo box' input type. Because this input type was not supported in the ArcGIS Enterprise 10.8.1 version of the ArcGIS Field Maps web application, here are a few potential workarounds or solutions:

Upgrade the version of ArcGIS Enterprise to 10.9.1+.
Add the feature services hosted by ArcGIS Enterprise to a web map hosted by ArcGIS Online. Then use the ArcGIS Field Maps web application through ArcGIS Online to configure the form accordingly.
Configure the form using the ArcGIS Field Maps web application through ArcGIS Enterprise 10.8.1. Edit the map maps JSON (using ArcGIS Online Assistant, etc.) and specifically the field elements within the formInfo section to change the input types to 'combo-box' for those contingent value fields.
