When you open a package from the active portal the first time, the package is downloaded and unpackaged. If you attempt to open the same package again from the portal and the portal item has not been updated, the package file is not downloaded and unpackaged a second time. You must delete the local cached version of the unpacked package at \Documents\ArcGIS\Packages to get a new version of the original package. All packages work in this manner, not just project packages. Each version of the package has a different GUID identifier. i.e. for projects, there might be two entries in the recent projects list, one that points to the old project and one that points to the new one.
