Error
A comma is added to the responses of a select_multiple question when the response to a previous question that is referenced in the relevant expression and the choice filter is changed in the ArcGIS Survey123 web app.

This is by design. The sample survey provided has a calculation on the select_multiple question that makes a non-existing selection when the answer to the previous question that controls the relevancy and the choice filter for the select multiple question is selected; the existing selection (which is blank in this case) is concatenated, separated with a comma, to the new selected choice.Solución alternativa
Add a hint to the question in the XLSForm to avoid going back on the responses, but to refresh the page instead if necessary.
