Bug
When a server object extension (SOE) or server object interceptor (SOI) is removed using ArcGIS Server Manager, the SOE or SOI is not fully unregistered from all machines in a multiple-machine site deployment.

Workaround
Remove or modify the C:\Program Files\ArcGIS\Server\bin\ServerTypesExt2.dat file.
If all the SOEs in the server needs to be deleted, the file in each server machine can be deleted. If only some SOEs in the server needs to be deleted, the file can be edited to remove some SOE tags by opening it with Notepad.
