"Item does not exist or is inaccessible" when submitting or viewing an ArcGIS Survey123 survey form

Description

When submitting or accessing a survey form in the ArcGIS Survey123 website, the following error is returned:

Item does not exist or is inaccessible.

Cause

A feature layer in the survey was changed and no longer has the same name as when the survey was created.

When a survey is created, the survey creates a folder in ArcGIS Online with the same title of the survey, and the feature layer associated with the survey. Any changes made to the title of the feature layer in ArcGIS Online affect the survey the next time it is being submitted or viewed.

Solution or Workaround

Rename any feature layer that was changed back to the name when the survey was first created.

Alternatively, publish the modified survey as a new survey with ArcGIS Survey123 Connect. Refer to ArcGIS Survey123: Publish a survey for more information.
