Is it possible to create custom map notes in ArcGIS Online?

No, it is not possible to create custom map notes or customize one of the map note templates in ArcGIS Online.

As of October 2014, there is no option to create a feature template or custom symbols in map notes. Feature templates define categories of items that editors are allowed to add to a feature layer based on specific combinations of attributes. Because map notes are graphics, they do not have attributes like feature services.
