ArcGIS Image Server 9.2 toolbar displays buttons as 'Missing'

Description

After upgrading to ArcGIS Image Server 9.2 Service Pack 2 or higher, the Image Service Editor toolbar displays with a number of missing buttons:
[O-Image] [O] missing_buttons

Cause

ArcGIS Desktop does not meet the minimum service pack required.

Solution or Workaround

Install the most recent or matching service pack for ArcGIS Desktop.

For example, if Service Pack 3 is installed for Image Server 9.2, then ArcGIS Desktop needs to be updated with Service Pack 3.
