Additional Information
This is an expected behavior in ArcGIS Field Maps. When configuring contingent values to support NULL value combinations and then configuring the same fields to be required through Field Maps Designer, prevents those NULL values from being entered into the field. This causes an invalid form to be created, which results in the message 'The form contains unsupported data.' in the ArcGIS Field Maps mobile application.
Again, this behavior only happens if the exact same field or fields support NULL in the contingent values and are then made required in the form.
Workaround
Make the fields to be not required.
