When a CSV file is added to an ArcGIS Online web map using the 'Add > Add Layer from File' method, saved as a feature layer, and published as a hosted feature layer, the field added using the Add Field dialog in the Data tab of the hosted feature layer item details page are not shown by default. This issue persists when the radio button is checked to show the field and the page is refreshed.

After review by the development team, it has been determined that this issue is related to a known limitation with the software that lies outside of Esri's control. The issue's Additional Information section may contain further explanation.
Additional Information
This is a known limitation and is working as designed. The visibility of a field in the table is controlled by the pop-up configuration. The Show/Hide option on the table does not persists and the visible state reverts back to how the pop-up is configured when the page is refreshed.
Workaround
Add the CSV file to ArcGIS Online from the My Content page. Publish as a hosted feature layer.
In the Visualization tab, click Configure Pop-up > Configure Attributes. Turn on the display for the newly added field.
