Using a search() appearance with a spatial query for a question in ArcGIS Survey123 Connect which references a feature layer hosted in ArcGIS Enterprise returns the error message "Failed to load the choice list. Please contact the survey owner to fix it." when filling out the survey in a browser.

Workaround
Publish the feature layer to ArcGIS Online and use the hosted feature layer in the search() appearance.
