This behavior is not reproducible with the latest version of ArcGIS Survey123 Connect. If the issue persists, contact Esri Support Services: https://support.esri.com/en-us/contactWorkaround
- Remove the input mask.
- Save the XLS form.
- Update and publish the survey.
