Additional Information
The Survey123 report capability has a known limitation where it cannot function and returns the error message, "An error occurred while saving the report template" if ArcGIS Enterprise deployments are not public-facing.

Workaround
Make the ArcGIS Enterprise deployment accessible from the internet to enable the Survey123 report to work as expected.
