The Simulate Overprint option does not print the overprint results while exporting to a PDF.

Additional Information

The overprint option has to do with creating color separations, typically four different color plates are used in the printing press, Cyan Magenta Yellow Black (CMYK). So, overprint is specific to CMYK, and destination color space on map or layout should be set to CMYK. The issue described here is not a bug. Post export from ArcGIS Pro with the correct color model set, there is also an option to enable the overprint preview inside the Adobe product to see what the end results look like when the print operation is done. By default, it might not be enabled.
