Additional Information
The documentation, https://doc.arcgis.com/en/survey123/desktop/create-surveys/xlsformformulas.htm, states: 'When using the pulldata() function, your .csv column names cannot be blank and cannot contain spaces, hyphens, or other special characters.' When there are values in a column with no header, the pulldata() function does not work as expected.
Workaround
Delete all the data out of the specified field area in the CSV file.
