This is actually not a bug. Hot Spot Analysis is all about looking for clusters of high values and clusters of low values. It looks at each feature, calculates a neighborhood around that feature, and then compares that neighborhood's value to the global average. If the local average is significantly higher than the global average, it is a hot spot. If it's lower, it's a cold spot. So it is no longer about individual features, but rather about neighborhoods, and clusters of high values together. Thus, a feature with a zero can absolutely be part of a cluster of high values, and a feature with points in it could absolutely be part of a cluster that is not significant (or even cold).
