The KML files created with the Generate KML option in ArcGIS Enterprise do not honor the elevation settings when published from ArcGIS Pro.

Workaround
If the contents of the KML file are opened in Google Earth, change the properties to be clamped to the ground.
Note: This works in some instances, but it is especially tedious with a lot of data.
