The Homelessness Activity Reporter survey form in the Homelessness Risk Reduction ArcGIS Solution creates a new feature layer upon republishing the survey after adding or modifying choice lists within the XLS form in ArcGIS Survey123 Connect.

Workaround

Create a new survey in the most recent version of ArcGIS Survey123 Connect. Copy and paste the questions and choices from the original XLS form deployed in the Homelessness Risk Reduction ArcGIS Solution into the new XLS form. Publishing the new survey creates a new feature layer, but any subsequent changes to the XLS form updates the schema of the newly-created existing feature layer, and does not create an additional feature layer. The newly-created feature layer can then be added to existing web maps included with the Homelessness Risk Reduction ArcGIS Solution.
