This issue is not reproducible in the current version of ArcGIS Survey123. If the issue persists, contact Esri Support Services.Workaround
Map questions created in ArcGIS Survey123 Connect (e.g. geopoint) appear to honor extent and zoom level settings when embedded in websites.
