The error message: "Error: Initializing form.. Form Logic Error" is returned when editing survey submissions that contain choice names or labels using double quotations for a question using the calculation jr:choice-name(choice, 'question').

Workaround
Do not use double quotation marks in choice names or labels that are pulled into another question.
