The Create Map Server Cache tool and Create Cache Schema tool do not consistently activate all the scales to allow tiles to be created.

Workaround

Navigate to the Service Properties in ArcMap or ArcCatalog and drag the slider over to the remaining scales to allow tiles to be created for those scales.
