The configuration of the forms in ArcGIS Online is not retained when the Save Web Map option is utilized in ArcGIS Pro.

This is a duplicate of BUG-000153433: In ArcGIS Pro 3.4, there is a new option to keep existing forms stored in the web map. This is a known limitation for earlier versions of ArcGIS Pro; forms configured with other apps are removed after saving the web map. When the map is saved in ArcGIS Pro using the Save Web Map option, re-open the forms in ArcGIS Online and click the 'Convert pop-up' button. The configured forms are now displayed.
