The conditional expression to display a specific question and hide the others based on a previous answer is not working as expected, as none of the options show up when filling out the form.

Additional Information
Under the 'Form settings' section in the panel on the right side of the form builder window, there is a check box to preserve values when fields are hidden with conditional visibility rules. Check the check box and click OK to close the form. The form works as expected. If doing this in the API, use FormTemplate.preserveFieldValuesWhenHidden (https://developers.arcgis.com/javascript/latest/api-reference/esri-form-FormTemplate.html#preserveFieldValuesWhenHidden).Workaround
