Not reproducible for the current release of Collector for ArcGIS on Android version 17.0.2.Workaround
In the ArcMap map document (*.mxd) used to publish the map service, edit the name of any feature class/layer such that forward slash symbols ("/") are avoided.Steps to Reproduce
