The ArcGIS Survey123 field app does not automatically recalculate values for questions with relevant expressions when the form is edited in the draft mode if the calculationMode parameter is not set.

Workaround

Define the calculation expression for the field using the XLSForm syntax or ArcGIS Survey123's expression builder. Under 'parameters', use 'calculationMode=always' for the integer question.
