Issue is no longer reproducible at the June 2022 release of ArcGIS Online. If the described behavior is experienced, please reach out to Esri Support Services for an ongoing investigation.

Workaround

Ungrouping the layers solved the issue in ArcGIS Online 9.3 but not in ArcGIS Online 10.1.
