The Alter Field tool updates the alias of the feature class in the geodatabase but does not update the alias of the corresponding layer in the MXD. The layer in Table of Content still displays the old alias in the MXD.

Note that ArcGIS 10.8.2 is the current release of ArcGIS Desktop and will enter Mature Support in March 2024 (https://support.esri.com/en-us/products/arcmap/life-cycle). There are no plans to release an ArcGIS Desktop 10.9, and we recommend that you migrate to ArcGIS Pro. Workaround
Recreate the MXD after running the Alter Field tool.
Remove the layers from the MXD.
Re-add the layers from the SDE database connection.
