Survey123 for ArcGIS report (beta) does not display a check box if a custom template contains a dash in the checklist.

Workaround
Navigate to the Survey123 for ArcGIS website and create a report using the attached (revised) template: AJB CIC Daily Inspection Report (revised).docx
The dash was removed and the checkbox displays correctly.
