Survey123 Connect for ArcGIS does not allow users to create surveys if they are not connected to the Internet or if the unsecured connections to ArcGIS Online are not allowed through the firewall.

Workaround

Download and add the templates to Survey123 Connect for ArcGIS. Create a new survey of them each time.
