Additional Information

The alignment issue is no longer reproducible if using the latest Survey123 Connect for ArcGIS 3.5 to publish the survey. There is a related issue to be fixed on the Survey123 for ArcGIS, which the default language set in Survey123 Connect for ArcGIS is not honored. This issue is planned to be fixed with the User Conference release.

Workaround

Use the language switch button available on the Survey123 for ArcGIS web form to change the language.
